By purchasing products and/or attending classes, workshops, webinars and meetings, I hereby agree to the following:
- I am participating in classes or services during which I will receive information and instruction about yoga and health. I recognize that yoga requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
- I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any physical fitness program, including yoga. I represent and warrant that I have no medical condition that would prevent my participation in physical fitness activities. I certify that I am capable of performing physical exercise and acknowledge that I am voluntarily participating in any yoga/meditation classes or workshops offered by Mindful Oasis LLC.
- In consideration of being permitted to participate in the yoga/meditation classes or workshops, I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which I might incur as a result of participating in the program. I understand that I will be fully responsible for complying with any restrictions prescribed for me by my personal physician and that I agree to consult my personal physician for further evaluation and such medical care as I require.
- In further consideration of being permitted to participate in the yoga/meditation classes or workshops, I knowingly, voluntarily, and expressly waive any claim I may have against the instructor, Mindful Oasis LLC,
the owner, or the class/workshop sponsor, for any injury or damages that I may sustain as a result of participating in classes or workshops.
In consideration for my voluntary participation in any yoga/meditation classes or workshops offered by Mindful Oasis LLC, I, my family, heirs, executors, representatives, administrators, and assigns do hereby waive, release, and forever discharge Mindful Oasis LLC, from any and all responsibilities, liabilities and lawsuits, present or future, and causes of action for ordinary negligence, whether foreseeable or unforeseeable, arising out of or related in any manner directly or indirectly, to my participation in classes and/or workshops.
I have read the above release and waiver of liability and full understand its contents. I voluntarily agree to the terms and conditions stated above.
